BORROWING DIAGNOSTIC MATERIALS FROM ISTODIEREVNITIKI

The records held by ISTODIEREVNITIKI, and in particular paraffin blocks and slides, can be used to:

  • Obtain a second opinion from another anatomic pathologist/laboratory
  • Carry out further specialised examinations
  • In order to protect both the patient and the anatomic pathologist who made the first diagnosis, ISTODIEREVNITIKI follows a specific, internationally respected, procedure for lending records.

ISTODIEREVNITIKI’s procedure for lending records

  • The patient or their authorised representative or doctor contacts ISTODIEREVNITIKI to inform it about the request to borrow the materials.
  • The loan request must be notified at least two (2) working days beforehand in order to recover the materials from the records.
  • When the patient or their authorised agent arrives to pick up the diagnostic material, they must bring the DIAGNOSTIC MATERIAL LOAN APPLICATION FORM filled out with the necessary demographic information and contact details.
  • ISTODIEREVNITIKI will then fill out the appropriate information relating to the diagnostic material on the form.
  • The patient or their authorised representative should sign the loan form to take receipt of the diagnostic material.
  • ISTODIEREVNITIKI keeps a copy of the loan form on file as proof that the materials have been extracted from its records.

NB

  • Where there is sufficient diagnostic material, ISTODIEREVNITIKI hands over a representative part or copies of the material.
  • Where there is a limited quantity of diagnostic materials, ISTODIEREVNITIKI hands over all original materials. In this case responsibility for preserving the diagnostic material is transferred to the recipient.
  • Where the original materials are handed over, to ensure patient safety it is recommended that the materials be returned to the ISTODIEREVNITIKI records after the re-evaluation is completed.
  • In order to ensure patient safety and to comply with medical ethics, it is necessary to inform ISTODIEREVNITIKI about the outcome of the second opinion provided by the other laboratory/anatomic pathologist.
